2. Information about your medical and surgical history
3. List of current prescription and/or over-the-counter medications, including the dose and frequency
4. Most recent X-rays or other test results related to your condition

Drs Hogan, Levy, and Rabin accept most major insurance plans, but we ask that you please call our office prior to your appointment to verify that your insurance provider is still accepted. If your insurance company required a referral letter prior to your visit, please request the letter from your primary care provider. Co-payments and any additional charges not covered by your insurance provider will be the responsibility of the patient or guardian.
